Special Radiographic Procedures and Concepts gives the practicing radiographer, and the computed tomography, magnetic resonance, cardiovascular-interventional, and vascular-interventional technologist, a working review of the fluoroscopically guided and image-guided procedures in which something is introduced into the patient to make visible what plain radiography cannot see. Four integrated sections cover the foundations every procedure stands on: pre-procedure screening, laboratory thresholds, metformin and anticoagulant guidance, consent and the time-out, moderate sedation, surgical asepsis, the fluoroscopic imaging chain and digital subtraction angiography, injectors, needles, guidewires, catheters, and the Seldinger technique, contrast media, reaction recognition and management, and fluoroscopic dose management; the gastrointestinal and biliary tract, from the esophagram and modified barium swallow to the barium enema, pediatric fluoroscopy, ERCP, and percutaneous biliary procedures; the genitourinary, reproductive, neurologic, and musculoskeletal systems, from urography and the pediatric voiding cystourethrogram to hysterosalpingography, myelography, arthrography, and vertebroplasty; and vascular and interventional procedures, from arterial and venous access and diagnostic angiography through angioplasty and stents, embolization, thrombectomy, vena cava filters, TIPS, dialysis access, drainage and biopsy, hemostasis, recovery, and the quality practices of the interventional suite. Each section opens and closes with a patient, names a weakness in taught practice and the habit that corrects it, and collects its numbers in tables; the course ends with a twelve-step pathway for a femoral arteriogram from order to discharge.

Learning Objectives

  • 1Screen the patient before a contrast or image-guided procedure, including laboratory thresholds, metformin and anticoagulant guidance, informed consent, the time-out, moderate sedation and monitoring, and surgical asepsis
  • 2Describe the fluoroscopic imaging chain, digital subtraction angiography, the power injector, and the needles, guidewires, catheters, and sheaths of the Seldinger technique
  • 3Classify contrast media, recognize and manage contrast reactions, post-contrast acute kidney injury, and extravasation, and manage fluoroscopic dose for the patient and the team
  • 4Perform and critique the gastrointestinal and biliary examinations, from the esophagram and modified barium swallow to the barium enema, pediatric fluoroscopy, ERCP, and percutaneous biliary procedures
  • 5Perform and critique genitourinary, reproductive, neurologic, and musculoskeletal procedures, including urography, the voiding cystourethrogram and reflux grading, hysterosalpingography, myelography, arthrography, vertebroplasty, and mobile C-arm work
  • 6Describe the vascular and interventional suite from access through diagnostic angiography, angioplasty, embolization, thrombectomy, filters, TIPS, dialysis access, drainage and biopsy, hemostasis, recovery, complications, and quality practices
